I think Sam is still Sam too, and yes he is similar to Mystery Spot Sam but in Mystery Spot Sam was just masking his pain and feelings, as soon as he had the trickster in front of him he was begging for Dean back. He was very emotional. This Sam seems different in that he isn't masking the pain so much as simply not feeling it. Not just pain but all his other emotions are muted as well. He doesn't feel empathy and that is why I think he made that decision not to go to Dean. Because on an intellectual level it seemed like the right one but he didn't factor in Dean's pain at Sam being in hell because he cannot understand that right now.
