What I noticed is the book that he kicked to the floor as his feet flailed around is on the floor after he woke up. So did he knock it down during his 'nightmare' after he woke up or did it actually happen?

well sometimes when we dream, we move around as if our body is going through the dream, such as kicking, so maybe he did knock down the book during the dream.

I know he shouted for Dean (and Bobby) when he woke up from it, and then went looking for them.
He shouted something for sure. And I think it sounds like Cas.
Ok I just watched it again. Chain comes down, Sam goes up. He gasps for air, shouts something and when his cheek almost touches the ceiling he shouts Cas. Just before you hear someone laughing.

Aw, man. Laughing. That must be part of Sam's Hell. There is laughing buried in the mix when we see Sam burning in hell also (at the end of Unforgiven and I think when he was walking and had that burning vision again in the finale).

I agree with you on that. I just wonder why Sam shouted Cas. He was in the cage, it would be logical to shout the name of the person who causes the pain begging them to stop

maybe sam subconsciously knew cas pulled him out but didn't succeed in getting all of him. that could explain why he shouted for cas....to save him.

I didn't notice that! I will have to watch it again and check out that scene again!
I always thought that the worst torture they could do to Sam is for them to appear as people Sam knows and loves (i.e., Bobby, Dean, John, Mary, or Cas) when torturing him. That would be horrible!
My friend and I were saying that this clearly happened to Sam while in the cage, so I hate to think it was the image of Cas doing it! Poor Sammy!

I think you're right. In the first one, he was trying to convince himself it wasn't real. As to calling for Cas, I still think he was remembering when Cas told him he could have thrown him back into Hell.
So maybe he thought Cas had changed his mind and sent him back?
